Terminology Key
- MAC: Maximum Allowable Charge
- OON: Out of Network
- INN: In-network
- UCR: “Usual, Customary and Reasonable” refers to the maximum cost allowed for a procedure.
- Indemnity Plan: Also called traditional insurance. With this plan, the insurance company pays claims based on the procedure performed, usually as a percentage of the charges.
Pay only a percentage.
- Covered services are paid based on a percentage of the dentist’s fee. For example, if fillings are covered at 65%, you pay the remaining 35% of the cost. (You may be responsible for a deductible or amounts over the annual maximum.)
- Dental procedures are grouped into three types:
Type 1 — Preventive
- Exams, Cleanings, X-rays (Bite-wings)
Type 2 — Basic
- Fillings, Simple Extractions
Type 3 — Major
- X-rays (Panoramic), Oral Surgery, Endodontics, Periodontics, Crowns, Bridges, Dentures, Implants
- To determine what your out-of-pocket costs will be, each Type (1, 2 and 3) has a pre-set percentage that represents what your plan will pay for the services in each Type. For example in the chart above, 100/65/20 coinsurance means your plan will cover 100% of Type 1 services, 65% of Type 2 services and 20% of Type 3 services.
